HOW TO REFER
The fastest way to refer a patient is by fax using our New Patient Consultation Request form. Once received, our correspondence team will contact the patient directly to schedule their appointment.

URGENT REFFERALS
For urgent retinal evaluations, please call (812) 334-1213 and ask to be connected to our triage team. We accommodate same-day evaluations for urgent cases.
OUR COMMITMENT TO CO-MANAGEMENT
We follow formal Medicare co-management protocols for post-operative cataract care. Your patients are returned to you for co-managed post-operative visits in accordance with Medicare guidelines.
